MSK Community Assessment Day (CAD)

Community Assessment Day (CAD)

If you’ve been invited to one of our Musculoskeletal (MSK) Community Assessment Days, please find more information on this page.

The CAD will be held at the Wades Centre, Stratton Road, Princes Risborough HP27 9AX. To ensure you’re fully prepared, here’s some essential information to help you make the most of your appointment.

What is the MSK Community Assessment Day?

We’re hosting a one-day event where you can meet our team of physiotherapists and specialists, all in one place. It is designed to:

Provide quicker access to expert care.

Reduce waiting times by up to 5 weeks.

Help you manage your condition with personalised advice, exercises, or a treatment plan.

Connect you with community services for extra support.

Many patients need only one session to start self-managing their condition, but we will create a plan tailored for your needs.

What to expect on the day

Your appointment will last between 30 minutes and 2 hours, depending on your needs. Here is how the day will go:

  1. Arrival: Your arrival time is your appointment time. You will receive a Patient Passport to help guide you through the day.
  2. What matters to you; 1:1 conversation: Start here to discuss what matters to you and your treatment goals.
  3. Access to services: Based on your needs, you may receive:
    • Personalised advice and exercise plans.
    • One-to-one physiotherapy.
    • Health checks (e.g., blood pressure, glucose screening).
    • Access to information like smoking cessation, diabetes, arthritis, or weight management.
  4. Community hub: Where you can meet with services such as Citizens Advice, Versus Arthritis, Healthy Ageing, Bucks Mind and income support.
  5. Final check-in: Before you leave, meet with a clinician to ask any final questions and confirm your next steps. Your Patient Passport will be scanned to update your personalised plan.

If you need further tests like scans or X-rays, we will arrange appointments for a later date.
